Unionline Access Request Form
Page one of the Access Form must be signed by an individual with authority to bind the company. The form is also used to designate any agent a customer wishes to access their Unionline data. All authorized users (i.e. contract holder company employees, agent employees, etc.) and access limits must be listed on the form. Once completed, the form must be signed and returned to Union Gas.

Agreement for Use
This agreement identifies the terms and conditions for Unionline use. Once an authorized user receives their login name and password and logs in to Unionline, the terms and conditions for Unionline must be read and accepted by clicking the "Accept" button prior to accessing the information on Unionline.
